Ask What a Quote Actually Includes Before Comparing Prices
A private medical bill can include the doctor’s consultation fee, tests, medication, the room, the operating theater, and other services. Asking only for the consultation fee or the daily room rate makes it hard to estimate the total cost of a course of treatment.
When comparing quotes, ask each provider to itemize the same service. For something as simple as a health screening, whether the lab tests, the doctor’s interpretation of the results, and any follow-up visit are included will change what you actually pay.
Consultation Fees Are Regulated, But That Is Not an All-In Price
Private clinics and private hospitals are not covered by identical fee schedules, and the items each schedule applies to are not the same. Under the amendment to the private clinic fee schedule that took effect in April 2026, a general practitioner’s consultation fee is set between RM10 and RM80, with separate arrangements for consultations outside normal service hours.
That RM10–RM80 range applies only to the consultation itself. It is not a cap on the total cost of a single visit. Medication, lab tests, procedures, and other services can still be billed separately, and the figure does not carry over to specialist care at a hospital.
When requesting a quote, ask the provider to break out the doctor’s fee from facility charges, tests, and medication, and to confirm whether any part of the bill cannot yet be estimated.
A Hospital Stay Quote Is a Budget, Not a Final Bill
The number of days in hospital, test results, the treatment approach, and whether intensive care is needed can all change the final bill. Surgical packages also depend on their specific conditions and what is included, as well as how complications or an extended stay are charged.
| Item | What to Clarify |
|---|---|
| Doctor and specialist fees | Whether consultations with other specialists are included |
| Room and nursing | Room type, number of days, and any extra nursing care |
| Surgery and tests | What the package actually covers |
| Medication and supplies | Whether these are billed separately, and whether there is an estimated range |
| Post-discharge costs | Whether follow-up visits, recovery, and take-home medication are included |
A Deposit Is an Advance Payment, Not a Cost to Add on Top
A hospital may ask for a deposit, a guarantee letter (GL), or both. A deposit is money paid to the hospital in advance, and it should be offset against the actual charges when the bill is settled; how quickly any balance is refunded or collected depends on the hospital and the insurer.
For example, if a budget already lists the full bill, adding the deposit again as a separate “medical cost” double-counts it. That said, a deposit still ties up cash while it is held, so when the payment is made and what the final cost is are two separate questions.
Even with medical insurance, you may still need to cover a deductible, items that are not covered, or pay some costs upfront before being reimbursed. Having a guarantee letter does not mean every final charge is automatically covered.
Situations That Are Often Misunderstood
“The daily room rate times the number of days in hospital is the total.”
The room charge is only one part of the bill. The doctor, surgery, tests, and medication can all be billed separately.
“I paid the deposit, so I still have to pay the full bill on top of that.”
Check whether the final settlement statement has already deducted what was paid. The same payment should not be counted twice.
What to Do Next
If your treatment plan is already set, ask the hospital for an itemized estimate, then confirm with your insurer what is covered, whether pre-authorization is needed, and what you will have to pay yourself. It is also worth setting aside funds you can access immediately for the deposit or for costs that run over the estimate. Medical quotes and insurance claims still need to be confirmed directly with the hospital and the insurer.
Frequently Asked Questions
Is the cap on a GP’s consultation fee the same as a cap on the total cost of a visit?
No. The consultation fee range does not cover all medication, tests, and procedures, and it does not apply to every type of medical service.
Does a hospital package guarantee there will be no additional charges?
Not necessarily. It depends on the package’s conditions, exclusions, and how the actual treatment unfolds.
If I have a guarantee letter, do I need to bring any money at all?
Not necessarily. The hospital may still require a deposit, and policy deductibles and uncovered items are still your responsibility.
